Addressing envelope to couple with different last names

Hello,
I'm addressing an envelope to a common-law couple and their little girl. The couple each have their own last names and the little girl has the same last name as her father. Nobody uses a hyphen. Should I address it to:
The Smith/Jones Family
The Smith-Jones Family
Tom Smith, Mary Jones, and Maggie Smith
...or other?

    Ms Mary Jones and Mr. Tom Smith --- on the first line Miss Maggie Smith --- on the second line.

    If they are married, it's:

    Ms. Mary Jones and Mr. Tom Smith
    Miss Maggie Smith

    If they aren't married, each person is on a different line.  I'm not sure what you mean by "common law."  Most states no longer recognize common law marriage, and requires people to hold themselves out as married anyway.  It's become synonymous with couples that choose to permanently cohabitate without marriage in some circles. 

    You never address an invitation to "The _____ Family."  It sounds like you are too lazy to be bothered to call them by name, and invites confusion.
