  Whatever he's interested in. Stuffed animal, movie, book, etc. I think that's a really sweet idea.

  The older children each got a little goodie bag with age-appropriate items.

  I gave my niece a couple of little books when her brother was born.  They weren't too expensive, so not a huge gift I think $2.99 each??), but she loves books and loved getting a present too at the same time

  A family I used to work for just had their second daughter in may so when I was shopping for the baby shower I got the baby something and I ordered a craft type kit(handband decorating) for their 5 yr old. They were ecstatic that I had thought of her and not just the baby, because during the time they did the baby registery she was asking why she couldn't put things for her on there.
  
  Maybe coloring books, crayons, color wonders? Something to keep him occupied but something where he can make pictures for mom, dad, and new baby.

  When I had my 2nd son a lot of family gave my oldest books about being a big brother, coloring books, and legos.
