  ETA:  You also just don't have to disclose you're a doctor; I've never met a doctor who didn't make sure everyone knew they were a doctor.

  There is a touch of "If you don't want to be publicly known as a doctor don't talk about it publicly ad nauseum."
  
  But if everyone knows just say, "Oh you know after 5 o'clock I'm doctoring a charceuterie board."

  [Not a doctor] but I never out myself to strangers. Other people do it for me or I try to side step and they get so nosy that eventually I end up answering. "Oh, I would schedule an appointment with your PCP to discuss your concerns." If I see something that I feel is potentially life threatening I might say something and still suggest ER or PCP depending on how acute it is, but that pretty much never happens. I will never understand people who go up to strangers and get up in their healthcare business.
