Formal invitation - envelope addressing questions

I swear I read 10000 topics on here about this, but as soon as I am trying to make up my address list, I can't find any of them!

Is the below correct? We are not doing inside envelopes.

Married couple but with different last names (I have one a few who kept their maiden names, one hyphenated, and one where she kept her maiden name but both are doctors, if that matters) - her first, then him on the second line, with and?
Ms. Maria Thomas
and Mr. Mark Dawson

Couple living together but not married - two separate lines, her first, no and?
Ms. Maria Thomas
Mr. Mark Dawson

Couple + child - their names on 1st line with and, child's name on the second line also with and?
Mr. and Mrs. Mark Dawson
and Elizabeth

The first two situations you have, you have it right, but with the married couple, I'd try to get their names on the same line if you can.

The couple with the child should be addressed as follows:

Mr. and Mrs. Mark Dawson
Ms. Elizabeth Dawson

We don't have any judges, colonels, or reverends. That I know of...

Yes, MD's. So the word "doctor" is spelled out and not abbreviated Dr.?

All is correct except that they go on the same line - not two separate lines. For the ones who are doctors and married with separate last names, it would be:

Doctor Maria Thomas and Doctor Mark Dawson (all on the same line)

Correct - and implies marriage. Generally it's correct for the woman's name to come first, but I agree with @Schatzi13 that if you're sending it to Mark's address, you should put him on the first line simply to avoid potential confusion.

No "and" on the second line for the kid. If you want to be super formal, use Miss and Master for girls and boys respectively. Also, use the kid's last name.

I thought of another question - what if the husband is Jr.? Where does that appear, if at all?

And I do have a PhD to invite... is it acceptable to use Mr. or Doctor?

The "Jr." would appear after the husband's name.

For the Ph.D., does he go by Dr. socially?  If so, use that.  Otherwise, call him Mr.

Yep

Dr. can be abbreviated when it is in front of a name on an envelope.
