    Oh, right! Good timing, since I'm actually on my computer.

    Attached is the edited version (with identifying information deleted). It appears also that the version on my computer isn't the most up-to-date, because it's missing the hymn numbers, which were in there, and missing the note we put in about not taking photos during Communion.

    But this is the more-or-less complete version, and it should give you a good idea.

    ETA: Fix attachment fail.
