Opinion on response card proof?

This is the proof I got back from Wedding Paper Divas for my response card. 

Right before I ordered, I asked them to add a line about dietary restrictions. I'm worried it makes the card look bulky. Maybe I should remove the bottom line and say "Please indicate any dietary restrictions on the back" instead?

Also, the meal selections are kind of a mouthful. I would have just said stuffed chicken breast, but I wanted to mention sausage in case anyone doesn't eat pork (though you can't taste the pork at all).

Thoughts? I wasn't even going to include the line about dietary restrictions - I figured those who had one would know to let me know. But my mom thought I should add it.

I don't think it looks bulky or too wordy at all.  I also prefer when the couple provides a bit more detail for the entree choices so that I actually know what I am getting rather then just guessing, so you are good there.

I find that response cards don't need to look super pretty because they are just a tool to get the info you need and for your guests to provide you with that info.

True, I'm just being psycho about it. I'm just worried I'm going to get back a bunch of dietary restrictions back like "no mushrooms" - unless it's an allergy, I'm ignoring it sorry.

I agree.  Food preference is 100% different then "I will die if I eat X, Y and Z."

And I agree with you -- dietary restrictions are things that will cause health issues (shellfish allergy, gluten intolerance, lactose intolerance, etc.). It's not a restriction that I don't like mushrooms, it's a choice. I can (and frequently do) just pick them out.

Yep, my venue can provide gluten-free, kosher, vegetarian, & vegan meals (and probably others). But I don't think they're going to make a special version of the chicken selection that doesn't have stuffing in it because you don't like pork. Order the salmon instead.

Looks very nice.

The only change I'd make (and this is just me) is to spell out the word "and" rather than use the & symbol.

I would suggest putting the dietary restrictions wording all on one line (if possible). And remove the line that people can write on. I think that will make it look less bulky.

Also, I think it's good that you indicated that there was sausage in the chicken dish, because there are many people that don't eat pork.

Yeah, that's why I included it. When we went to the tasting we weren't even going to try that dish because of the pork. But we tried the other two chicken dishes and they weren't that great, so the made that one and it was by far the best. If I knew it would be a major problem, I could get the stuffing made without pork (for everyone) but I don't know too many of my guests who don't eat pork. My sister doesn't like pork, and she thought it was fine, and my kosher friend said he would order the salmon anyway.

Too late, I already approved it. I thought about fitting it on one line, but I'm not sure it would fit.
