addressing Dr on a thank you card

Do I have to, or can i just write her first name. If I saw her , I would call her by her first name ( she is FH cousin) but I wasnt sure the proper way, or if Thank you cards had to be as formal as invites.

If you or your FH are on a first-name basis with her (and I presume your FH is because she's his cousin), then you can use Dear FirstName as the greeting.  You address the envelope to Dr. FirstName LastName.

Outer envelope:
Dr. First Surname

Card:
Dear Cousin First name

Blah blah blah

JEN and HISGIRL are correct.  DH and I are both doctors, and the envelope still gets addressed to Dr.  But the card inside just has first name in the salutation.

Wait, I never knew that thank you cards have to be addressed formally. You can't just first/last and omit the title?
