I'm inviting my cousin and his ex who my family is still close with. On which of their invitations should I include their daughter?

Who does she live with or stay with more often?  If it is even, I would invite her on cousin's invite.

I agree -- invite her with the custodial parent OR the parent who will have custody of her the weekend of your wedding.
