Online RSVPs - Custom Questions

Hi everyone,
I want to start collecting RSVPs for our wedding but cannot find an option to add follow-up/custom questions within the RSVP. Does anyone know how to do this?

I am having the same issue. My friend was able to do it, but it is not showing up as an option when I log in. Hopefully someone has some insight in how to fix this!
