Wedding Cake Toppers

I want to have a monogram of three initials on the top of our wedding cake.  What is proper etiquette for a monogram?  Should the first initial of the bride be first ... the second initial is larger and an "R" because that's the first initial of my fiance's last name ... then I'm guessing that the third initial would be the first initial to my fiance's name ... Is this right?  I want to order this monogram/initials (letters) soon so I want to make sure that I do it (order) right.   (Oh, just overall wedding etiquette ... Should the brides first name be first on everything ... e.g., invitations, Save the Dates, website, programs, Thank you cards, etc?)   Thanks!!!

Is this right?   <em>For a traditional monogram, the groom's intital, the joint last name, the bride's first initial.  Personally, I think this is outdated.  A single initial of your joint last name is more elegant. </em>

    Traditionally, the first letter is the bride's first name, the second letter (and biggest) is the first letter of the last name and the last letter is the man's first letter of his name.

    So, for example: my first name is Faith, my FI's name is Max, and our last name will be Collins. So, our monogram would look like this:

    FCM (except the "C" would be larger).
