Wine bottle poem tags template

Hi all! Does anyone have a cute template for the wine bottle poem tags (first fight, first anniversary, etc.) to go on bottles as part of a shower gift? I have seen a PDF template but it wasn't editable and I wanted to change some wording/personalize without starting totally from scratch.

You will need to retype the poem (make it personal, its more fun that way).  Looks like you would have to make labels anyway.  anyway google is your friend:
