Question ideas for "guestbook"

((I posted this on FB, too. But want as many suggestions as I can get))

We are desiging little booklets to put on the tables before dinner so that guest have something to do and fill out while they wait....like this:


Now I need some more suggestions on questions to put in it. We are doing a mad lib and some trivia pages. But would like some more questions to add to the book that are fun and also some questions are more advice driven. Let me know if you have any ideas for me!! TIA!
