Show me your RSVP wording?

Im trying to get the RSVP set in stone for our designer. How are you ladies having your guests fill it out with a food choice. I need to know exactly who wants what. Beef, Chicken or Fish I know this is gonna be a PITA making sure they fill it out right.

I'm still trying to figure out how to add our website RSVP info on here since it is password protected. Basically we will give them the option to mail the response cards back (we will not pre-pay postage), or direct them to the RSVP page on our website to RSVP there

Please respond by April 1st

You may RSVP two ways
By voicemail (202)xxx-xxxx
or
By the wedding website

<div>That's what I put, too. But I should have said how many seats were reserved for them. So for the others that I have yet to send out, I'ma put how many seats, so ppl will stop RSVP'n for an extra person.</div>

Ok, MsApril I think Im gonna take your idea and tweek it lol, I am definitely using how many seats are reserved for you.  SInce you are now done with this process did peope fill it out right or check mark chicken etc?

I have no idea what my own RSVP cards will look like because I haven't really done much in the way of planning since my wedding is a ways off (June 2012). However, I came across this photo a while ago and thought it would work well if I need to know my guests' entree choices in advance. The checkboxes on the left are for people to let you know whether they're attending or not. The checkboxes on the right are for guests to select an entree. Maybe you can use something like this.
