Remove the "at" before the time, and capitalize the h in half.  Capitalize the "b" in Boulevard and remove the comma after boulevard.  If Rxxxxxx stands for the town and state, it should probably be on a separate line.  

Looks good to me otherwise!

 I think the phrasing is awkward.  Maybe you could change it say something like 'Because you, our amily and friends, have shared in our love Amber & Mathew...." and the rest is fine excpect for the changes above.
