Address Help Please!

Finalizing the address on my STDs and I've ran into a problem. One of my cousins is a young single mom with two boys. How do I address her? For families, we are just addressing them to "The Smith Family". Not supper formal or proper, I know but that's how our wedding is. I would do this for her too, but the boys have their father's last name. So should I address it as "Ms. X and Family" or "The X Family"? 

FYI: STDs and invitations only have one envelope. So I need somehow to convey that the boys are invited on the outer envelope.

I would address it as Ms. X, boy #1, boy #2.  That way it's clear that her boys are invited.  I think propper ettiquette is:

Ms. Jane Doe

Jack and Joe

10 Wedding Street

Wedding Bells, New York 00000

I'm not 100% sure, but I think the kids' names go on the 2nd line.

I'd say:

Ms. So-and-so, Mr.boy 1 and Mr. boy 2

or

Jane Smith, Tom Jones and Nick Jones.

I think the second one is best.

Just FYI...Young boys get addressed as 'Master'.
