I have a question about the marriage certificate that I am not sure if you can help me with. We were a little overwhelmed at the time and We left the part blank that says Mr. _____________________________ of _______________________________ Ms. _______________________________ of __________________________________ Is this supposed to be filled out by us, the officiant, or by the county once we send it in??? It really is not clear. Everyone signed who needs to. If we fill it out, I assume I put my maiden name at this part? Thanks for any help! Leslie
