How to address a Bishop?

Our officiant is a Bishop, and I was wondering if anyone knew the following:

How do we address his invitation?
How do we write his name in the program?

    The Most Reverend is the proper way I believe.

    A bishop is addressed as Your Excellency

    Let me check my book when I get home later, and I'll get back to you. I think it follows the way you would address other clergy. For example, I am addressing my invite to Monsignor as follows:

    Outer Envelope:
    The Right Reverend Monsignor Firstname Lastname

    Inner Envelope:
    Monsignor Lastname

    For Bishops I believe it is "Most Reverend", but it might go as follows:

    Outer Envelope:
    The Most Reverend FIrstname Lastname, Bishop of X

    Inner Envelope:
    Bishop Lastname

    I'll double check for you tonight, though!

    KatieM - I tried to google it and one website said to Most Reverend First Last, Bishop of x...but other websites left off the Bishop of x part, so I wasn't sure.  Also wasn't sure about the inner envelope.  So if you have a resource to check I'd really appreciate it!

    Ok, so according to Crane's Wedding Blue Book:

    Outer Envelope:
    The Most Reverend FirstName Lastname

    Inner Envelope:
    Bishop Lastname


    I think you add the Bishop of X part on the programs, but if you attend Mass at the church where the Bishop presides regularly, you could look at one of the programs there for the wording.
