Is there a standard prayer of the faithful or do we need to write our own? I know one thing I would like to include in it is a mention of my father, who died 25 years ago. Is there a standard one where I could just add that in?
There were standards in the book they gave us (I am blanking on the name) but I liked some of those and also wanted to include some of my own so I am picking and choosing which ones, which the church is ok with.

The book is probably "Together for Life". It can be found on Amazon if your church doesn't give/lend you a copy.
There are several standard ones that you can choose from, among them a prayer for those who have died. The wording is something like "For those who have died, especially ___". Our priest told us to feel free to write our own if we wanted to. I think we did tweak some of the ones in the book but stuck to their intent for the most part.
Yeah, our parish gave us a copy of Together for Life (which also made picking readings a snap). We made some modifications but went with the general theme of the lists in the book.
Our priest wrote ours. But it sounded pretty standard, pretty much out of Together for Life. He did mention the names of the people we lost (at our request). A special one for me and DH, and one for all married couples or soon-to-be married couples, as well as some of the traditional ones you hear at Sunday Mass.
The deacon who assisted at our Wedding Mass wrote them up! Our parish had a standard baseline that they went from and he added some intentions at our request including the prayers for the poor, victims of injustice, the unborn, our engaged friends, and the military (fi's sister in the army and couldn't attend wedding).

For those who are oppressed by society because of their differences, that we might live in a more just and equal society that allows us to see and value the humanity in others, we pray to the Lord.
For all affected by the current economic troubles, that all families are able to provide for their children and that all unemployed workers find dignified work at a living wage, we pray to the Lord.
For all Christians, for the Jews and the Muslims, our older and younger sisters and brothers in faith, and for people of all faiths and no faith, that we might live together in harmony and mutual respect, we pray to the Lord.

For all of our silent intentions (pause), we pray to the Lord.
For our Holy Father, Pope Francis, that The Holy Spirit may continually bless him with the graces he needs to boldly proclaim Christ as Lover to the ends of the earth. Let us pray to the Lord.
For The Church, may she always be an example of how to actively seek the Heavenly Bridegroom amidst the distractions of our busy world. Let us pray to the Lord.
For world leaders, that they may be inspired by the Holy Spirit to be advocates for the forgotten, the vulnerable, and the unwanted; May their actions always promote the dignity of life and peace in our world. Let us pray to the Lord.
For the sick and the suffering, especially those suffering with mental illness, depression, or addiction; may they be blessed with insight so that they may begin to find peace, rest, and hope in God, the Wonderful Counselor. Let us pray to the Lord.
For Amanda and James as they start their life together, may they be blessed with a deep and passionate love for God, for each other, and for every child they welcome into their home; May God grant them strength, hope, and gratitude in every moment of their journey together. Let us pray to the Lord.
