Escort Cards XP

How did you ladies put guests names on the escort cards?  We addressed our invites formally as Mr. & Mrs. John Smith but I'm thinking for the escort cards I'd rather do John & Sally Smith.  Does it matter?  Who's name would you put first?  Do you always put the man's name first or do you put the person you know better (ie Sally is my sister) so would I put Sally & John Smith?

We did individual cards for each person, and put John Smith on them. I didn't want a super formal feel, so I felt that was okay.

Our invites and wedding were very formal so we put "Mr. and Mrs. John Smith". But I think it's perfectly fine to go the informal route. For my sister and her date, though, I did list her first as Ms. First Last and Mr. First Last. I'm not sure if there are specific rules to this though.
