Passport name change help

Hi all,

I've finally gotten around to changing my name on my passport but I'm stumped on one of the questions. It asks for date and place of the name change. Did you put down the date and place of the marriage, or the date and place of the social security office when you actually handed in the paperwork? Anyone know?

    hmmm good question...I put the date I was married, and the town where I got my marriage license, but now I'm thinking I'm wrong...Good think its sitting in a pile so I can still change it once there are more responses!

    I believe I used the date that DH and I were married.  That's technically the date that began the reason you can change your name.  Because you have to put a copy of the marriage certificate in there with the application, I think that's the way to go.

    agreed, use your date of marriage and the town you got your license in.

    I just filleed out my paperwork for this this morning.  I put down the date of our marriage and the town we got married in, which is the town we got our marriage license in.  We are going on a cruise this summer and I need it for that.
