Hey! How did/are you wording your RSVP cards?

I've come across a thousand different ways and I'm not sure which way we're going yet. I am also considering making them a little less formal...something funny maybe. I found this idea and thought it was too cute!!....

 was worried that some people would bring a guest even though their invite was to just them no & guest wording, so my rsvp is as follows:
M__________________
 ___ seats have been reserved for you

___ attend
___ decline

The favour of a reply is requested by ___.

This is formal but could be changed around to be informal. We are filling in their names and the number of seats that have been reserved for them.

Guest Name (I typed it in so there would be no add ons.)

That's really cute but I think it's totally confusing and I would only use it if you don't care whether or not your responses are accurate. People are already retarded enough with their RSVPs then to give them four will attend options/

Also, I just wanted to add that one time I posted an event on meetup and the response choice was will be there with bells on and several people (who were kind of retarded) emailed me seriously asking if they had to bring a bell and what it was for and why everyone was responding that they would wear a bell.
