changing of last name forms??

To my already married knotties:

where do I get the paper work to fill out to change my last name? I want to work on this asap. And what else do I need to fill out. Completely lost.

You need to go to social security to apply for a new SScard.  I think they have a form online you can print and take with you so you don't have to do it there.

Then you go to the DMV with the paper that SS will give you and get your new license.

Yeah, the Social Security office is your first stop, then DMV. With your new license, let your bank know you've changed your name so they can give you a new ATM/debit card, also your credit card company.

Aggree with PP.  Also, look carefully at the DMV website because they have recenlty changed their requirements.  There is additional identification that you need to bring with you.

well from experience last week when i went to the dmv u uhave to have ur new ssn card with ur new name.

The new proof of last name in order to change is thru social security and ur passport. ur passport has to have ur new name. until u have that u cant get ur lisence.

i know its a crap

oh and 2 proofs of current address

thanks for the helpful info. Printed a form to get my SSN card and from there I will go to the DMV. thanks again

oh wait u cant go directly from the ss place to the dmv u have to wait a day until the social security place processes ur ss and then the next day they give u letter with ur new name to take to the dmv but i would advise to check with ur passport with ur name cause if not they wont give it to u
