PLEASE HELP: RSVP wording

We are going to have a sit down meal at our wedding and we have the option of 5 different meal choices. I keep thinking of how to effectively use a small amount of space on our RSVP card to be sure we have names and meal choices for everyone so we can make place cards. Recommendations/ideas or any input on what people have done that has worked well is very greatly appreciated!!! Thanks!

Here's what we did for multiple meal choice options:

We have reserved _____ seats in your honorThe favor of your reply is requested before xx/xx/xx
M____________________________________ Accepts______ RegretsIndicate each guest’s entrée selection with initials
___________ Chicken
___________ Beef
___________ Pasta
___________ Children’s meal

I have a picture of mine in my bio under DIY. We listed all the menu options at the bottom of the card and then had each person's name listed next to different check boxes where they could select their option.

We handwrote a quick note about veggie or kid's meal options to the few people where that was relative -- that was easier for us since we had a small-ish wedding. If you chose to set it up this way, it would be easy to just add a couple more options.
