Passport Name Change question

I'm reading up on how to change my name on my passport.  I talked to a lady who gave me the forms and she said to send in the original marriage license, but in the document/form she gave me it reads that you can send the original or a photocopy.  What did you do?  How much was it for a name change?

    You can try calling to make sure, but I think you need the original or a certified copy.  (Not just one that you copy on your own.)

    Also, I think you have to pay for a whole new passport if yours is more than a year old?

    It says you need the original or certified copy of your marriage license.

    Then, the fees: If it's more than a year old, you have to pay the normal first-time application fee. (boo.)  But do it soon! I heard passport fees are going up later this month.

    You have to send an original or certified copy (ordered from the state) in with your application.  You will get it back though although it comes in an entirely different envelope than your new passport so don't freak.  I totally did and if someone can learn from my panic, then I don't feel so bad!  :-P  It was quite a bit of money to get a new passport, I did mine when it was time to renew it and it cost me more than a hundred bucks although I don't recall the exact total.

    Ditto this.  It has to be a certified copy (or the original) - not a photocopy you'd make at Kinko's.  I had to pay for a whole new passport, new pictures, etc. since mine was issued in 2004.
