Addressing the outer envelope

What is the proper way to address an outer envelope to:

My female cousin (last name #1), Her boyfriend (last name #2), Her child who has a different last name (last name #3).

Any help would be appreciated.  I'm at a loss!

    I would write it like this:

    Mr. Boyfriend Lastname and Ms. Cousin Lastname

    Then if you have an inner envelope, add the cousin's daughter on there. If you don't have an inner envelope, write it like this:

    Mr. Boyfriend Lastname and Ms. Cousin and Cousin's Daughter Lastname

    I just had something like this recently. I'm pretty sure I read that it should be
    Line1: Ms. Cousin Herlastname and Mr. Boyfriend Hislastname
    Line 2: Miss Daughter Herlastname

    I think for married couples, the man's name goes first but in this case and as it is the female that you are more familiar with, her name goes first. Children always go on a separate line if you are going to include them on the outer envelope.

    According to etiquette it should be worded as follows:

    Ms.Cousin Lastname
    Mr. Boyfriend Lastname
    Miss. Daughter Lastname
    Address
    City, State, Zip

    You shouldn't include the 'and' because that indicates marriage and unmarried couples always have the female listed first.
