Foccus survey?

I have to take the Foccus Survey on Tuesday at our parish. Can someone please give me a hint as to what the questions are like?

    there are different sections....finances, children, religion, sex, family etc
    you answer the questions based upon whether you agree, disagree, or unsure.

    They ask you if you are pregnant, lol.  And some weird sex questions.

    DX: PCOS/Recurrent losses/MTHFR mutation (compound hetero)
    5 hysteroscopies/2 surgical
    3 Inject IUIs = 2 m/c's and 1 BFN
    IVF #1= BFP. m/c at 7w6d. Needed 2 D&C's and scar tissue removal. Mild OHSS
    IVF #2 = BFP. Severe OHSS. 4 Drainings. TWINS!

    took one for the Catholic Church, not sure if it's the same.  But they sat DH and I down w/ our #2 pencils, and the test was about 100 questions long.  I felt like I was taking the marriange SAT!  Keep in mind that some of the questions can be interpreted very differently so don't freak out if you guys don't match on some that you thought were obvious.  He may just have read it differently.

    </div><div>Just hit me really wrong.  Anyway, it's not that bad.  Like the other ladies said, questions on finance, raising a family, religious beliefs etc.  They just kind of ask the same questions over and over again.</div>
