Wedding Address Etiquette

Ok, so I think I have most of the etiquette down, but I still have a couple questions that I found conflicting answers to online and wanted to see what you all had done! 

1. If a couple is dating and living together, do you:

a. put your closer friend's name first
b. put the guy's name first
c. list them alphabetically
also, they go on separate lines, right??? 

2. if a couple is married and the woman kept her maiden name, the names go on the same line and the woman's name goes first, right??

    1) I have seen both. If one is obviously your friend and the other is just his/her significant other but not necessarily your friend, go with A. If they are both your friends go with C. And yes, they always go on separate lines with no AND in between.
