I'm not sure what you are talking about specifically. But our photo and DJ had forms that we had to fill out.

They asked our names, addresses, phone numbers.
And then the wedding stuff:

starting point (for photog)
where bride was getting ready and a phone number
where groom was getting ready and a phone number
photo begins where -list place and address
ceremony site: place name and address and contaxct and phone number
reception site" place name and address and contact and phone number
special picture sites? before ceremony? in between ceremony and reception?
what time is ceremony, cocktail hour, reception
who is getting introduced / special people
bridal party names and relation to us
first dance song
parent songs
cake cutting song
ending song
what time does reception end
Anyting "special"  going on

It was mostly stuff like that. At least, thats what our DJ and Photographer wanted to know. Not sure about Jake, but hope this helps! You are so close! Congrats!
