Name Change NYC

Hi, sorry if this is posted elsewhere, but what do you need to bring with you to do legal name change with the Social Security office in Manhattan? Do I need my birth certificate? I have certified marriage certificate, passport, drivers license, old social security card—is that enough? Thanks so much.

    I brought my marriage license and that was enough.They did take away my old SS card.

    yes that is enough! the SS office in the theater district seems to be not as busy. when i went i only waited 20 min to be seen. you take a # from a machine when you walk in and then wait for the # to be called.

    Marriage certificate plus a gov't issued photo ID with your old name (driver's license or US Passport).

    I went to the office on 48th between Broadway and 8th yesterday and I only waited about 10 minutes.  All they asked me for was the completed form (which they have there) and my marriage certificate.

    Oh my gosh... I went to the office near city hall on a weekday at 2pm and waited 2 hrs 15 mins!!!

    I went yesterday to the Staten Island's office and as soon as my number was printed they called it.  I was there for maybe 4 minutes total!
