Has anyone applied for a name change on their passport lately?

I can't figure out where on the application to use my maiden name and where to use my married name. Help?

Use your current name except for where it requests the name listed on your current passport.

I did this about a month ago... although my passport was over 1 yr old, the forms are pretty much the same.

Like Milkduds said, your new name goes at the top of the form asking your name, while you put your maiden name or other names you have used in the past at the bottom above the photohead shot space.
