address invites

What is the proper way to address the following cases (all married couples)?

1. both medical doctors with same last names
2. both doctors but different last names
3. wife is a doctor
4. husband is a doctor
5. couples with Phds

    Outer: The Doctors Smith OR Doctor Jane Smith and Doctor John Smith
    Inner: The Doctors Smith

    2. both doctors but different last names
    Outer: Doctor Jane Smith and Doctor John Doe
    Inner: Doctor Smith and Doctor Doe

    3. wife is a doctor
    Outer: Doctor Jane Smith and Mr. John Smith
    Inner: Doctor Smith and Mr. Smith

    4. husband is a doctor
    Outer: Doctor and Mrs. John Smith
    Inner: Doctor and Mrs. Smith

    5. couples with Phds
    Very old school etiquette says that PhD is a professional title and therefore unless the invitation is for a professional event, the person is addressed socially as Mr. or Ms.  However, if you want to recognize them with their doctorates, they would be as you would address married doctors.
