Response card wording

Hi there!
I'm working on my response cards, but i'm having a bit of trouble. We're having a sit-down dinner and plan on using different place cards to indicate to the wait staff whose ordered what entry. I remember my friend had to call to ask couples who wanted what entree for her wedding, but I'd like to word the response card so I won't have to make too many calls later, any suggestions?

This is how I had ours done. You'll still probably have to make a few calls, some people just aren't good at following directions.
