Our venue printed ours and left them set up on each place setting.

Ask your venue. Ours does not provide menus.

Ours provided them. We decided to do our own to match our invitations. It was DIY and not too expensive.

Ours provided them. I didn't even know this until the day of, and was going to DIY but ran out of energy. Luckily, the design they did was pretty darn close to what I would have done so I was extremely happy.
