I'm not doing this, but have been to several weddings with donations, and they all had individual notes on the place settings

I have seen it many ways.. and the most effective way I think is one at each place setting.

We had a section on the bottom of our programs that said 

"Thank you all so much for your love and support, and for sharing this day with us. In lieu of favors, we have made a donation in honor of our guests to one of our favorite charities, the Humane Society of New York." It didn't seem right to spend extra money to "advertise" our donation -- we wanted the money to go to the cause (and also didn't want the cause to spend money to make something to advertise our donation).  And we didn't mind if people missed the message.

My cousin had it printed on the cookie they had as a favor.

The last wedding I attended had the message attached to a cookie. To me, the cookie would have been fine as a favor alone, so it ended up that there were 2 favors, IMO.

we didn't do it, but my cousin put a sign in a nice picture at each table, next to the centerpiece.  i think this reaches a wider audience than putting 1 sign somewhere; and, it's less of a hassle (and less costly), than put one  at each place setting

I am putting a note on the back of my placecards explaining the charity and why we chose to donate to it.

I got little boxes and filled them with Hershey Kisses with a note explaining that we donated to 2 Children's Charities.  It was more of a favor/donation.

I put it on the backside of each of the escort cards.

We are doing table frames with the charity and why we donated. It's cheaper to do one per table and I think it's less clutter on the tables.
