How to word this invite?

  So I'm am currently addressing my invites and I have a dilemma.  FI's cousin recently got remarried.  Me, FI, FI's sis and husband all know because we saw pics on facebook.  We asked her about it recently and she told us the truth.  She told the rest of the family that the pics were a joke to piss off her ex husband.  I don't believe she changed her last name to his and we are also inviting her 2 daughters (11 and 14).  How am I to address the invite??  I was thinking of doing it like this:   Danielle Her Last Name and Roger His Last Name and sending a seperate invite to the daughters.  HELP!!

    What does the back story have to do with addressing the envelope? She's just a married woman who hasn't changed her name. (Though I would ask her how she prefers to be addressed if you're not sure.)

    Then list the children on the inner envelope.

    You could say:

    "Ms. Fiance's Cousin's Name", & Guest & Family

    or

    Ms. Fiance's Cousin Name & Family

    I was about to ask how to address the invite if you dont have inner envelopes (i.e. we are doing the pocketfolds, so we won't have an inner envelope.)
