Being asked if you are pregnant?

Has this happened to anyone before? How did you respond (and not be sassy)? 

I've been asked this 2 times before, and each time I totally died inside. Both times its been by patients, so I can't tell them how rude it is to say that. Bewarned: venting. I just hate it when people ask this. I know they don't mean to be rude, but it is. And  when someone says that I just keep thinking about it over and over. I am SO insecure to begin with, and this just really sucks. Even when I have patients ask me if I have children, I assume they think I look pregnant. I know they are just making conversation, but I can't help it.

Once, by my mother of all people.  Couple years back, I was visiting, had a short shirt on, so when I went to stretch my arms up, my mom saw my stomach.  She looked at me and said "omg honey are you pregnant?"  I said, "no mom I'm just fat, thanks for noticing".  The rest of the evening went by with me pissed.  Then later on I told her I'm happy with my size, she's never said anything again.
