Response Card Wording

I feel like this is a total "brain-fart" and it should be easy but here goes:

How do I do the wording when we are serving a filet, chicken, and vegetarian. I need to know if it says 1 chicken, 1 filet, is John or Susie getting the filet?

I feel like it's a simple answer, but I've been thinking about it for WEEKS! Thanks Ladies!

We got an invitation that asked us to choose our meal option and I figured they might well need to know which of us ordered what so Fi and I initialled next to our choice.
