IDEAS for menu choice from RSVP card to place card

I am having a sit down dinner with three choices (Beef, Chicken, and Fish).  The hotel would like me to show the difference between the three on the place card.  On the RSVP card at the bottem I was going to have  ___ beef  ___ chicken  ____fish.
In the blank space the guest would put a number for what they would like.  So a family  of five could be  2   beef   2 chicken  and 1 fish. So here comes the question How do I know who wants what for dinner???  I do not want to put the wrong food item on the place card.

We asked for initials and got a few with numbers.  We just made assumptions of who ordered what and assumed that guests could switch at the time if needed.
