Do wedding and rehearsal invitations need to be mailed separately or can they be mailed in the same envelope?

You can include the rehearsal invite as an additional insert with the invite - that's fine

Strictly speaking, they should be mailed separately two weeks before the wedding.  Personally, I think the timing can be much more flexible, but I still think they should be mailed separately. 

My crew would forget to send back both RSVPs if they were in one mailing.

We did ours in our invitations and added an extra line on the RSVP card.  This really only works though if you're inviting everyone to your rehearsal dinner.

 If you're just inviting the traditional people to the rehearsal dinner, I would send seperate mailings.
