Rsvp cards, children invited, expecting not-entire families to attend

What's the best way to get guests to indicate how many family members, of several invited, are attending? For example, my cousin is married and will have 3 children by our wedding. They've already told us his wife can't make it, and I can't imagine him traveling alone with the kids, but of course we'll invite all 5 people. Does this work? We don't need to know about children's meals nor have individual names for a seating chart.

Please indicate numbers accepting and declining.
__Accept with pleasure
__Regretfully decline
R.s.v.p. by October XX.

More notes: We're doing letterpress, so we're ordering invitations early. We have overseas guests, so they'll go out on the early side, too. I have to look at the catering contract for the precise Rsvp deadline, but I think it's 3 weeks before, to get a headcount in 2 weeks before.

I think your wording is too complex.

Declines ________

Since you're inviting entire families including the children, you don't need to include the number of seats reserved for them.  I'm not fond of presuming to know the responders emotions - maybe they're accepting with regret, or declining with pleasure.

I did this:

 "xx" seat(s) reserved in your honor"

___accepts
___declines

My responders put the number attending in the accepts part if the entire party invited was not attending; like my friend put _2_ accepts, as I invited 3 people in the home.  People usually do that if not everyone in their group can attend.  If everyone on their card was attending, they put a check mark on the accepts line.

I didn't specify number of seats reserved, but wrote the names of all those invited and put:

M_____________________
__ accepts with pleasure
     number of adults ___
     number of children ___
__ declines with regrets

They should be able to figure it out! lol
