Address question

When addressing the outter envelopes do you write Mr. & Mrs. or Mr. and Mrs.?  Also, when inviting a guest do you wite Mr. Joe Smith and Guest or & Guest? Thanks!

For the married couple:
Outer - Mr. and Mrs. George Smith
Inner - Mr. and Mrs. Smith


For Joe Smith:
Outer - Mr. Joseph Smith
Inner - Mr. Smith and Guest

Thanks for the help. With my invitations they are a pocket fold so there are no inner envelopes. So do I just put and guest on the outer envelope?
