menu choice on RSVP

how should i indicate what the menu choices are so people know to check off which one they want?

If I just have

Etienne and Brianna  ____ accept  ____decline

i'm not sure where ask if they want chicken or fish. i want it on the RSVP cards and I'm doing it that way, so plese don't tell me it's not proper or whatever.

(Well actually I wrote a little more than accepts/declines but you get the idea. I personally don't like marking chicken without clearly saying that I'm also accepting. It is important to separate out guests; I had a number of people who only one member of a couple is coming to the wedding)
