Dinner Selection Cards

We got our invitations and they don't come with a dinner selection card.  We are having 2 selections which are prime rib or chicken.  I have to make the dinner selection card myself.  How should I do this.  I am not sure how to word it.  I was thinking something along the lines of this:

Please respond by June ? 2010
Name_____________
#Attending________________
#Regrets_________________
Chicken__________________
Prime Rib_________________

Please let me know what you think.
