Escort card questions

A few questions:
- Did you have yours printed or print them yourself? If you did yourself, what cards did you use/did you use a template? 
- Did you do full first and last names on them? I've posted previously about FMIL frustrations and how she refused to cooperate and give me formal names for invitations. I was totally mortified when I sent an invitation to "Jay" and I got an RSVP back that had "Jason" written on it. 
Is it totally inappropriate to just put "Mr. Smith" or "Mr. and Mrs. Smith" and leave off first names?

    I'm not doing escort cards but I feel you on the name thing. FMIL gave me M.A. L*********.
    Got RSVP back that Mary Alice D******* was attending (she hadn't even given me the right last name! I was embarrassed by that.

    I folded paper cranes and wrote Mr. John Doe and the table number on the wings.  I'd skip Mr. Doe, because people might get confused if their first name isn't on there (we have several families coming with the same last name, like when there are 2 Mr. Does).

    I’m printing mine myself.  I plan to put first and last (and table number), but you can do it any way you like.  I’m still trying to decide how I indicate which meal they picked.  It’s giving me a headache.

    My mother and I are hand writing the escort cards and I think I might do it however I'm feeling in the moment. haha More than likely I'm do John and Jane and then their first letter of their last name stamped. If there are two John and Jane's... well I'll cross that bridge when I get there but I don't think that's a problem.

    I had mine printed along with my programs for $40. I just did first and last names and plan to put them on table already and my aunt will have a list and as the guest come in she will tell them which table. Really not the way I wanted to do it but with 2 days left...IM SO OVER IT!

    We did ours formally so they say Jane & John Doe instead of Mr. & Mrs. If you do go formal I would put Mr. & Mrs. John Doe incase there's another Doe family, KWIM?

    I used a label template for ours and am printing them at Staples on cardstock we purchased. I forget which template, but we only needed flat cards so I don't know if that will help.
