Filling out the RSVP

I am filling out all of the RSVP cards with the guest name so all they have to do is check a box if they will attend or not. How do I write couples name? Mr. John and Jane Doe, Mr. John and Mrs. Jane Doe? And what if I only know the wife or only the husband, can I place whomevers name I know most first? It is also a casual wedding. Any advice will help, since I'm doing just about everything on my own and DIY.

You probably need to do a check box for each person in the couple.  It's not uncommon for a person to come without their spouse, especially if the spouse doesn't even know the couple.  

You always put ladies first.  It doesn't matter who you know.

As far as ladies first, I've read it both ways, although most on here say ladies first.  If my FI is inviting his best friend and BF's girlfriend, we're putting the best friend's name first.
