How to word "special meal requests" on RSVP card?

I want to give my guests the option of requesting special meals for dietary needs (allergies, gluten free, vegetarian, etc.).  What is the proper way to word that on the RSVP card?  I want to avoid "I want steak please".  lol

Can you just do, "Dietary Restrictions:" and that's it? if some one lists steak, I would assume its a joke.

I was thinking writing "Allergies/Dietary Restrictions: ______________" and ignore the silly requests.  I wasn't sure if there was a better/more appropriate way to phrase it.

We are having a buffet so we only need to know about people who would need a plated meal to avoid contamination.

We included a line that said, after listing meal options and a place to write the names of those requesting which option, "Please indicate dietary needs along with dinner choice".

With a buffet, I think a similar statement would suffice "please indicate any dietary needs, next to guest's name", or something of the sort.

I'd just do a dietary restrictions line, and then follow up with those who list allergies. For example, I have a tree nut allergy, which I would list. But if you followed up, I'd let you know that it is (luckily!) not so severe that I can't eat at buffets. I just need to know which dishes have nuts.
