S/O Name Changes

I'm also going this week to get my name changed. I've printed the form and gotten my documents proving my identification and citizenship pulled.

So my question is:

If I want my full legal name to be jessandtrav middle maiden married do I write the middle and married in the middle name box?

  • That's a good question.  I'd wait until I got there to ask and fill it in on the spot ;)
  • You mean middle and maiden in the middle name box? If so, yes that's what you do, or at least that's what I was told to do at the Social Security office. She went over all the options for the name change and how to fill out the paperwork before I sat down and did it. I ended up doing an easy old fashioned name change though :)

  • I think I'll just leave it blank. I'm considering just being old fashioned too, Whit. I know I want to change it, and that's about all I know. ;)
  • Yes, you would write your middle names in the middle name box. Because you will be giving yourself 2 middle names.

    This probably isn't true everywhere, but I know at least in KY, you can't do that.  I went to get my driver's license renewed the other day, and stood behind a woman trying to do that.  They wouldn't let her, and said it's a post 9/11 thing.  Your middle name has to be either the middle name on your birth certificate or your maiden name.  You couldn't keep them both for a middle name.

    The woman had already had her SS card changed to say First Middle Maiden Last, and was telling the people at the DMV that her license won't match her SS card if they don't let her make that change, and they said their hands were tied.

    The woman threw a SHITFIT, asked to speak to a supervisor, and he told her she would have to go to probate and appeal to a judge if she wanted to keep both as two middle names.

    Just wanted to warn you of that, although it could just be in my state.

In California, you can change your middle name to your birth last, current last, spouse's birth, spouse's last, or a few others. And you can change your last name to the birth name of either spouse, the current last name of either spouse, a hyphenated combination of any current or former last name of either spouse, or a combined last name of both spouses. (Example: the mayor of Los Angeles was born Antonio Villar. When he got married to his wife, Teresa Raigosa, they combined their names to Villaraigosa.) So basically, almost any combination you can come up with. Though if you want a completely new last name you have to get a court change. And they tell you all this on the marriage license form, and you fill it out then. </div>

    I've decided that if it's an issue, I'll drop my maiden name and keep my middle name.

    I like all of my names, but I just really like my middle name.
  • Yes, this is what I did. It gets a bit confusing at times especially when filling out forms that ask for your middle initial (since I have 2 middle initials). But I'm very happy I kept my maiden name, since I love it so much
