Name Change -??

Does anyone have steps/directions on how you go about changing your last name? I don't know where to start.  I could have sworn I saw a checklist or something somewhereAlso, we're getting our marriage license on Friday - how many certified copies do I need?

The certified copies won't be available until after the officiant sends in the signed copy.  Then you can go to city hall and get the certified copies of it.I just got one copy and I showed that to Social Security and DMV and they were fine.If you need to change your passport I would get a 2nd one, because they need a copy of that one as well.HTH

I google and found this.... Start with... Social Security AdministrationApplication for a New Social Security CardNote: It takes about 10 days for IRS records to be updated after the Social Security Administration effects your name change. Department of Motor Vehicles - Drivers License and Vehicle RegistrationThis is likely to require an in-person appearance at your local Department of Motor Vehicles to submit forms, which are provided on the premises. Bring two kinds of identification, such as a certified copy of your marriage license and/or birth certificate, your current driver's license, or a U.S. passport. Don't expect to be able to change the name or address on the title of your car, however: A new title is only issued upon transfer of ownership. Continue with... I only got 1 copy of the marriage certificate... gave it to the priest and then he turned it in to the town && then they mailed us our finalized copy. First I changed my name on the SS card. I took like 5 mins at the Meriden office and then I changed the name on my license. Both places just made a copy of our marriage license and they gave me back the original. Then I did the credit cards/bank accounts little by little. The credit cards I just did over the phone - They didn't need any info.. I just did it verbally. For the bank accounts I just had to show them a copy of the marriage license and they made a copy of it for their records && again returned the original to me. Here's what it says online for what you need for the SS office && DMV to change your name: SS Office: What if my name changed? If you legally change your name because of marriage, divorce, court order or any other reason, you need to tell Social Security so that you can get a corrected card. If you are working, also tell your employer. If you do not tell us when your name changes, it may: Delay your tax refund; and Prevent your wages from being posted correctly to your Social Security record, which may lower the amount of your future Social Security benefits. If you need to change your name on your Social Security card, you must show us a recently issued document as proof of your legal name change. Documents Social Security may accept to prove a legal name change include: Marriage document; Divorce decree; Certificate of Naturalization showing a new name; or Court order for a name change. . If the document you provide as evidence of a legal name change does not give us enough information to identify you in our records or if you legally changed your name more than two years ago, you must provide Social Security with additional documentation. http://www.ssa.gov/pubs/10002.html#identity DMV: Change of Name If your name has changed, you will need to go to your nearest DMV Full Service office, Satellite office or Photo License Center to update the information on your driver's license and vehicle registration, if necessary. Below is what will be required to change your name for each type of transaction: Change of Name on Photo License: A completed Change of Name Request (form E-78). Current driver's license. Certified documentation that shows the change of name (i.e. marriage license, civil union certificate, divorce decree, probate court documents). Photocopies and religious marriage certificates will not be accepted A new license will be issued at no cost, indicating the new name. Note: In a situation where a license has been lost or stolen, the above documentation is required along with the documents and fees that are required to replace a lost, stolen or damaged license. http://www.ct.gov/dmv/cwp/view.asp?a=805&Q=244738 HTH!

I went to pick ours up when we got back from the honeymoon on that monday.  So we had it 9 days after we were married. They told us it really depends on how fast it gets sent back to them.

You have to change SS first because they need to see your license as proof of your ID. The only other thing you have to bring with you is your certified marriage license. If you are changing both you license and registration, you need to get the new license processed first. You can do both on the same day but you need the new ID to get the registration changed to your married name. It doesn't cost anything for either and they print the registration right there. You just need to call the credit card companies but I would do them in stages because you can't use your old cards once you tell them to change it. HTH!

Our officiant sent in our marriage license 2 days after the wedding and it was ready for pickup in New Haven about a month later. We weren't given the option to have it sent to our home. We had to go there and pick it up. We got 3 copies - 1 to keep, 1 to send away for name change stuff if needed, and 1 that I carry in my purse for whatever may come up.

I'm in the process of doing this now. I changed my name at the SS office, but I haven't done my license yet. My banks and work only required the new SS card or marriage license, not drivers license. I had to change my Bank of America and credit union accounts in person (both of us needed to be present for joint accounts), but I was able to mail a copy of my marriage license to ING (same with student loans). That's as far as I've gotten. I'm lucky that work will inform all of my benefits providers for me!
