For my BM, to I address the invite to them, then their H...or do I always put the male first? Also if two people live in the same houe, grandma and uncle, do they get one invite or two?

Actually, unless you are writing "Mr. and Mrs. John Smith," the woman always goes first:
Ms. Jane Doe and Mr. John Smith
And all guests who are over the age of 18 should get their own invitation, unless they are a couple, in which case two people go on the same invitation.
Married, different last name: Ms. Jane Doe and Mr. John Smith
unmarried but dating/engaged, living together: Ms. Jane Doe Mr. John Smith
unmarried adults and not a couple, living together: if they're siblings, you can do "The Brothers Smith" or "The Sisters Smith" otherwise, each person who has graduated from high school gets their own invite
with children: Mr. and Mrs. John Smith (or one of the other above options for unmarried OR diff last names) Miss Jane Smith Mr. Tyler Smith
I'll add that if you decide you want to list any married couples who have the same last name with both of their first names, which I am probably going to do in at least some cases, it would be:

And you can always skip titles and do: Jane and John Doe
To my mind, any of those is is vastly preferable to Mr. and Mrs. John Smith.

I think the objection here is that you're not supposed to separate a man from his last name, no? But maybe that's silly.

BTW, on my invitations for my first wedding, for spacing reasons, I did actually do what you suggested:
Mrs. Jane Doe and
Mr. John Doe
but I've never seen anyone else do that.
