Name Change Kit?

I've been so bogged down with work and moving I still haven't changed my last name!  Is there something like a checklist or something of all the things I need to change? I thought I seen a post about this before but, can't find it. :(

    Did you mean where you have to change your name? If so this is everywhere I found I need to change it at:

    • Social Security / IRS (Internal Revenue Service)
    • State DMV (Department of Motor Vehicles
    • Voter Registration
    • Passport / Post Office (name and address change)
    • Electric / Gas / Oil (for heating)
    • Cable / Satellite TV
    • Telephones (Land lines and cells)
    • Water / Sewer (may be combined with water) / Trash pickup
    • Internet connection provider
    • Mortgage / Equity loan holder
    • Landlord / Building management
    • Property titles / Deeds
    • Auto loan / Lease holder
    • Title / Registration
    • Vehicle sticker for local municipality
    • Roadside service
    • Employer (through HR department)
    • 401-K / Insurance / other benefits plans
    • Business cards / Business email
    • State Bar
    • Unions / Professional associations
    • Business licensing agencies

    or just add your new last name and you can use both and only change a few things:)  much easier lol

    Yeah my list is a little long I found it on a website and I didnt go thru and take condense them. It seems like a lot but I think some are kind of obvious ya know

    lol it's not as bad as it looks though!  once you do social security & DMV, your name is officially changed.  the rest you can do over time...i've been married over a year and there are still places i haven't changed my name with...

    wow. That list scared me! lol  There was something somewhere that you could get that had everything like with the applications etc and checklist for where you needed to change..hmmm I dont know if I'm even thinking of that right as to what it has. I'm actually taking the day off next week so I can go to DMV and  Social Security.  And I completely forgot about my passport!! Thanks for the info!
